Participants must read the CONTEST RULES before sending in the application:

Individuals must submit one application per entry and pay a registration fee of $25.

For minor contestants, a parent/guardian must fill the form/s, agree to abide by the the rules of the contest and accept responsibility for the participation of the contestant.
